Overview

The Title IV Authorizations form collects required authorizations from students via an interactive Smart Form available in the Student Experience portal. All Smart Form responses are surfaced within Regent Award under the Student Portal Authorizations section in the Student Details tab.

Title IV Authorization Smart Form

Document Requirement

The Title IV Authorization Smart Form is configured with the Title IV Authorizations Smart Form Type on the Document Setup → General tab.  When a document is configured with this Smart Form Type and the document is added as a document requirement, the Title IV Smart Form will be accessible on the Documents component in the Student Experience portal. Smart Form documents can be configured to require an electronic signature but it is not required.

Activity Log Entry

An Activity Log Entry is created when a student completes the 'Title IV Authorizations' Smart Form or a staff member updates an existing Authorization in the Regent Award user interface.  The Activity Source will indicate if the Authorization was updated from the completion of the 'Title IV Authorizations' Smart Form or from a staff user in the Regent Award user interface.

Displays the configured question text from REM configuration, 'Title IV Authorizations Description' located in Institution Setup on the Portal SmartForm sub-tab under the Title IV Authorizations Settings for the default Institution.

Default Text: 

Federal student aid is used to pay for current tuition charges and mandatory fees associated with attending {SchoolNameDisplayValue}.  Upon receipt of each disbursement, {SchoolNameDisplayValue} will apply your financial aid funds to your tuition and other allowable charges such as books and other course materials. {SchoolNameDisplayValue} may also retain funds to pay for future tuition charges and mandatory fees associated with attending {SchoolNameDisplayValue} with the student's permission.  In addition, students may incur other educationally related fees while attending {SchoolNameDisplayValue} such as graduation fees, etc.  Federal student aid may be used to pay for these expenses with the student's permission.  Please note that if you do not authorize federal student aid to be used to cover these costs that you will be responsible for covering these fees using non-federal aid funds.

If you decide to change or cancel your authorization, the change will be in effect for future disbursements and cannot be made retroactively.  Please provide your authorizations below:

Default Text: 

I authorize {School Name Display Value} to retain credit balances resulting from Title IV funds (such as Pell, SEOG, TEACH Grant, and Direct Loans) for use in paying future institutional or other educationally related charges that I may incur. I understand that by selecting this option I will not receive a refund for any credit balance for which I may be due unless I specifically request the refund at a subsequent point in time.  This authorization will remain in effect throughout my education at {School Name Display Value} unless I revoke it by updating the authorization to ‘no’.

If 'Display Authorization to Retain Title IV Funds' configuration is enabled in REM, this question will be shown and required.

This question will default to 'No' on an initial application and will prepopulate with the student's responses on subsequent applications.

Displays the configured question text from REM configuration, 'Authorization to Return Federal Loan Credit Balances to Lender' located in Institution Setup on the Portal SmartForm sub-tab under the Title IV Authorizations Settings for the default Institution.

Default Text: 

I authorize {School Name Display Value} to return federal Direct Loan credit balances on my account to the lender in order to reduce my loan debt.  This authorization will remain in effect throughout my education at {School      Name Display Value} unless I revoke it by updating the authorization to ‘no’.

If 'Display Authorization to Return Federal Loan Credit Balances to Lender' is enabled in REM, this question will be shown and required.

This question will default to 'No' on an initial application and will prepopulate with the student's responses on subsequent applications.

This question will default to 'No' on an initial application and will prepopulate with the student's responses on subsequent applications.

Displays the configured question text from REM configuration, 'Authorization to Pay Non-Institutional Charges' located in Institution Setup on the Portal SmartForm sub-tab under the Title IV Authorizations Settings for the default Institution.

Default Text: 

I authorize {School Name Display Value}  to use my excess federal financial aid funds (such as Pell Grant, SEOG, TEACH Grant, and Direct Loans)  to pay any outstanding non-qualifying charges appearing on my student account for the current award year or loan period.  Non-qualifying charges can include health care, health center, university store, parking permits/fines, and other miscellaneous charges.  In the event that any amount I owe becomes past due, I agree to pay any and all late charges and /or costs of collection.  This authorization will remain in effect throughout my education at {School Name Display Value} unless I revoke it by updating the authorization to ‘no’.

If 'Display Authorization to Pay Non-Institutional Charges' is enabled in REM, this question will be shown and required.

This question will default to 'No' on an initial application and will prepopulate with the student's responses on subsequent applications.

Default Text: 

I understand if I decide to change or cancel my authorization(s) indicated above, a new authorization form will be required and the changes will take effect for future disbursements and will not be retroactive.

If 'Display Statement of Understanding' is enabled in REM, this question will be shown and required.

This question will default to unchecked and will not prepopulate from previous applications.
